Trailhead Physical Therapy · Early RTM pilot

From its own treatment plan to a billing file.

Trailhead brought its own protocol into BoneArc. Patients recorded their work between visits. The clinician reviewed and certified the RTM month, and the practice downloaded the billing export for its own filing process.

A clinic-authored plan, carried into patient monitoring and clinician review.

01 / The care plan

The protocol stayed Trailhead's.

Trailhead used its own treatment plan rather than replacing it with a generic program. BoneArc gave patients a place to follow that plan and record activity at home, while the clinician could review what happened between appointments.

That matters for an RTM workflow: patient activity, setup, education and clinical management need to remain connected to the care the practice actually prescribed.

02 / The first ten days

Three kinds of RTM work appeared in the record.

Within ten days of practice signup, Trailhead had captured setup and education, the first two MSK monitoring days and ten minutes of treatment management. Those correspond to three distinct 2026 CPT pathways:

98975Setup and educationPatient setup and education documented.
98985MSK device supplyTwo monitoring days recorded in a 30-day period.
98979Treatment managementFirst ten minutes of clinical management captured.

These are categories of documented work, not a claim that all three codes were submitted or paid. CPT 98979 also requires a real-time patient or caregiver interaction during the month; the practice must review that evidence before billing.

03 / The handoff

The clinic finished with a file it could use.

The clinician later certified the monthly RTM record. On August 25, the system exported two supported RTM lines. Trailhead downloaded the file on September 8 to handle filing through its own workflow.

The pilot demonstrates the path from a practice-owned treatment plan through patient activity and clinician review to a billing handoff. It did not measure payer submission, payment, time saved or clinical outcomes.
